The school-based nasal flu vaccination programme helps protect your child against flu. Flu can be a dangerous illness and sometimes causes serious complications. Your child is entitled to the vaccination free, from the NHS.

THIS FORM MUST BE SUBMITTED BY THE PERSON WITH LEGAL PARENTAL RESPONSIBILITY FOR THE CHILD (EITHER BIRTH PARENT OR AGREED THROUGH COURT).

Your child’s school has provided us with your contact details. We will update your child's electronic health record and notify your GP of the immunisation.
Your consent to vaccination will be valid for the period of our flu programme. If your child receives the vaccine elsewhere, or you withdraw your consent, please contact us.
