Dear Parent/Carer
Whitehouse Community Primary has been selected to take part in this year’s Dental Epidemiology Programme of Year 6 pupils. The programme collects information on the health of children’s teeth and will be carried out by a dental team visiting the school. Information regarding the visit is attached along with a consent form that needs to be completed.
When completing the consent form please fill in the details of your child at the top of the form. Only sign the form at the bottom if consent is granted for your child to be part of the Programme and for the dental team to have a look at your child’s teeth.
Only children with written consent will be seen by the dental team. They will be selecting a sample of Year 6 children so if consent is given your child may not selected by the team. We will be notified nearer the time how many children will be seen.
